New Eden layout planned, random, or what?

This is basically just a question for my own curiosity. I was wondering how the layout of New Eden was put together. Did CCP hand-craft each star system and its location, jumpgate connections, etc? Did they start with some kind of procedural system that generated regions in a more or less standard way? I know they've changed some things since the beginning, including adding new regions and reshaping the jump gate network around Jita, but I was wondering if anyone knew how much the layout of EVE was carefully planned and how much is just happy chance?

I don't know the details, but I think it was procedurally generated. I remember reading a while back (a fanfest report maybe?) that when they re-generated the final cluster after alpha, Hilmar changed the initial seed number to 42.

I think they also edited names, or had a black list, so obvious obscenities didn't get in.
