wormholes are portals to different places in EVE. most of them lead to unknown space (wormhole space) and some lead to other highsec, other nullsec, and other lowsec systems. they can be scanned down anywhere, and nowhere (location is completely random) and they have very powerful rats. actually sleepers are currently the hardest rats to kill in the game due to their insane DPS. there are 6 classes of wormholes, and different sites in the wormholes, but i shall not go into detail on that because it is far too detailed to explain here. instead you should look up "wormhole EVE" in google to find a wiki on it.

Wormholes are portals to solar systems far outside the reach of normal space travel. They can take you to either systems in an other region or to "wormhole space". Wormhole space contains rare ore and advanced NPCs. Players do not show up in local and security is 0.0. There are no stargates in wormhole space.

They work in a similar fashion as a star gate but are natural spacial phenomena. A Stargate acts as a man made wormhole. Wormholes destinations are often not plotted and thus can lead to dangerous regions of the cosmos.

To add to what has already been said - Wormholes are found by probing down the signatures. They can be found anywhere in Eve. If you enter a wormhole it is a good idea to take a probing ship and certainly bookmark your entrance... many people have spent days lost in wormholes trying to find a way out again! Entrances can also collapse so having a probing ship with you at all times is usually considered essential (to probe down a new exit if you current one collapses).
